We allow our employees a free copy of everything in their personnel file once a year if they request it. They have to pay for copies if they ask for it more than once a year. (Luckily so far I've only had a couple of requests over several years).

It really is state-by-state... According to Nevada Revised Statutes, a former employee can request a copy of their files within 60 days of termination of employment.

My understanding is if it was a "good faith error" you can date it to the start of the leave. There's some case law that upheld it and I was advised by our HR Legal Department of the same.
