This is my first experience with DOT Drivers. Do you have separate files for their DOT stuff (drug test results, copy of medical card, driving abstract, etc) or do you just keep it in their regular employee file?

I keep my DOT files separate. I purchased a Driver Qualification File from Ryder that has a checklist of what needs to be in the file and forms for you to use. See link below.
