Independent Contractor Form

Denise:
Just a small caveat----------many states require that such agreements contain specific language to ensure the agreement's enforceable (e.g. must have cert for w/comp; liability insur minimums), so you'll also need to be aware of your state requirements.

I'm assuming that you already incorporate the use of the W-9 form?

We require our IC's to sign an agreement that details a number of things, including non-employee relationship, property rights to the work they do, how they should submit invoices and what our turnaround time is, etc.
