Does anyone have any evaluation forms they are willing to share? I need to create a form that's in part a self-evaluation. Our VP wants me to create a preliminary form to hand to managers to set a personal development plan for next year. Categories to be included in this preliminary form are personal/professional goals (ex. bring in new business), training & skills development, safety goals, etc. These goals will be linked to their raise.
Has anyone done anything similar? I would really like to see some examples. Or maybe someone can give me some resources that I could use to create the forms I need. Thanks!

I can email you a couple of documents that we use if you provide your email address. They are a segment of an entire system but will offer some assistance.
