When is a LTI recordable with OSHA

Hello everyone,

I'm one of the new kids on the block with a small maufacturing company in the NW. A problem recently came to light and I need some help with it. An employee injured himself back in November of 2004 and had no days off at that time. He had surgery last week and was off work for three days. Do I record the "Lost Time Injury" as happening in 2004 or 2005?
