No Smoking Policy

Do any of you have a policy for Non-Smoking on office premises? I need a sample to go by. What we want to do is put a policy in place that allows a window of time to become effective thereby giving the smokers time to either quit or become adjusted to no smoking during the day unless they are off premises. We would also offer a smoking cessation program if needed. Thanks for any suggestions.

FYI: The Mississippi statute permits banning use of tobacco on your premises but forbids adverse job action based on "off premises use of tobacco".
