Mileage Reimbursement

Looking to se how other companies calcuate mileage for reimbursement to employees? We have some employees who leave from their home to work but may report into the office sometime during the day or may not report in at all. Do you calculate from their home less the normal miles to the office? Want to make sure that we are paying according to mandated guidelines.
We also have sales representatives that are paid a monthly allowance but if they go over a certain amount less their daily commute miles they are reimbursed an additional amount. Do you have a different plan for paying this type of employee?
Any help would be appreciated.
