Affidavit of Marriage for Health Insurance

A current employee has applied for health insurance for his wife. There is no marriage certificate as they were married in a country that does not commonly issue certificates.

We administer our medical plan, and have been advised that the typical solution is to obtain an affidavit of marriage from the employee and spouse with an acknowledgement that misrepresentation is cause for termination and/or recovery of costs.

Has anyone come across similar issues - have a sample affidavit?
