Employment Verification Request

I am being asked to complete an non-DOT employment verification for a former driver who is applying for a driving position. Normally I would receive a separate form that asks about drug screen history but the one I received does not. My concern is we terminated this person for a post accident drug screen that happened during his first 90 days. Am I required to disclose this information if they are not specifically asking for it? Thanks!
