Performance Appraisals

My company is considering adopting a new performance appraisal form with a section that posts a graph showing the employee's actual score for each factor and what the manager believes their potential score is. I'm concerned that some employees will think the "potential" score is unfair or discriminatory; it seems very subjective to me. Does anyone else use this type of rating? If so, any problems or complaints from employees?
