I would like to get input from my employees about there current options for Healthcare, I would like to conduct a survey that will be blind and get their true feelings without them feeling like there will be backlash if they express themselves.  I am wondering does anyone have a survey that they have conducted in the past that could assist me with this.

    Just make sure they can submit their responses anonymously and you will get honest answers.

     

    Open ended questions will give you rich answers, scale answers will give you easily measurable and possibly useless answers.
