An employee was terminated for falsifying medical insurance paperwork. What are the laws for getting reimbursement back for the medical insurance (such as time limits etc.) and will the medical insurance company need to press charges (and the time limits etc.)?

Probably falls under theft of services. I doubt you can get back the employer's portion of premiums without a lawsuit. In some places you can deduct from a last paycheck if you can "prove" that they stole. The insurance company will have to pursue their own interests. You could report it to the DA and see what he or she says.
