Wow, Terrific summary.  I guess my only suggestion would be that there are a number of companies offering something between a phone interview and a face to face interview or structured interviews and that is a video interview.  These can be live or scripted and recorded. This is often precedes a face to face interview where distance and travel to a home office is a consideration. Don't you just love technology!
