Thus far I have presented questions on this forum but this area I can assist. I put together a rather elaborate excel based tracking system for FMLA (regular and new regs both), attendance and other occurrences. The attendance is based on your policy but could be adapted to fit your needs/policy. Also, it includes instructions on how to use it. All that you (whoever tracks it) have to do is imput the data and this spreadsheet will track all occurrences and it will indicate if you are to give an warnings/terminations for attendance and it will inform you when the FMLA (the regular and military caregiver leave) will expire.

I was quite surprised in dealing with ADP, PayChex and others a couple years ago that none of them had any FMLA component to their time and attendance system.
The response here suggests that remains a major problem, particularly in the SMB space.
