policy on using employee images for recruiting and other company advertising

Does anyone have a policy that describes the acceptance of an employee for the Company's use of their image(s) for recruiting and other advertising for the Company,, both while they are an employee and beyond their employment?

I've thought about it in the past, but we really weren't using employee images until recently. Now, it seems like I'm posting an employee photo on Facebook at least once a week. I guess it's time to add a waiver to our new hire docs.

Our Dir. of Marketing has a release form that employees must sign before we use their photo in any printed or electronic means. Such as events on our intra-net or for the newspapers, phamplets, or calendars, etc...

In my former life at a NFP we had a form, no policy that I recall. Maybe your attorney could help? Or a search?
