ours has at best been wasteful of time, energy and resources. we are in the process of moving physicals from the er to the family practice clinic. does anyone have a good form they are willing to share

Sorry, lerobb, our's are provided by the clinic performing the physical and appear to be proprietary. You might ask the clinic if you can see what they use.
