Intermittent FMLA

We have an employee who has been approved for intermittent FML for taking his seriously ill mother to her doctor appointments. Since appointments are usually scheduled in advance, we have requested he give us a week's notice before any appointments. This morning he called in saying he was busy with his mother, but we have reason to question if that is really why he did not come in. It is very disruptive to the operations in his department when he is not present. Is it OK for us to require a note from the doctor confirming any unscheduled appointments? If he is really with his mother and she needed his help, we understand. We're just not sure that is the case.
