CA Employers - Short Term Disability?

We have 3 employees who reside in the state of California, even though we don't have an office there. One employee wishes to exercise his rights under the CA short term disability benefits, but, as fate may have it, I have no experience with this.

I tried finding the appropriate government website, but can't seem to load any of their pages. Do any CA forumites out there have the state disability claim form? If so, would you mind e-mailing it my way at [email]sarah.hulsey@risingms.com[/email].
