Release form for references

Does anyone have a reference release form that they use on departing employees that they would offer to share with me? Thanks, Jody

    We don't use a release form, but we tell the employee during their exit interview that if asked, we will only give "name, rank & serial number" for reference checks unless the employee gives us their express permission. As for that express permission, we accept either a written statement or e-mail from the employee consenting to disclosure of information, or a signed authorization from the requesting entity. For example, if I receive a request from a background check company, they are normally accompanied by a signed release, and that will suffice.
