payback agreements

We have a situation where an employee has been overpaid incentives due to error on client's part. The overpayment is substantial. We would like to spread the repayment out a little, but believe an agreement should be signed first to protect us in case the employee should suddenly leave. Does anyone have a repayment agreement I could modify for our use?
