Manager Reviews & Employee Feedback

We are changing the way that perfomance appraisals are done and want to implement something like "peer reviews"; but, rather than peers, this would be an opportunity for line employees to give feedback on their managment. HR would select relevent employees and ask for feedback. This feedback would be used as a tool during the managers reveiw.

Can anyone offer advice on the type of questions to ask, or anything else we need to be sure we are thinking through?
