Hello... New to this forum... I am in the banking industry and would be interested in a performance review for hourly & salary positions. Can anyone help?

Also, can you put the consumer report and background check disclosure information on the back of a job application instead of a separate form?

Our background check authorization form goes to our BGC provider, so I keep it separate. I do include a check-off on the application that they acknowledge they will submit to BGCs if they make it that far in the process. That's where a lot of your real nutcases fall out.
