form to request PTO

We are updating our policies and procedures and would like to institute a form to be completed by an employee when requesting PTO (Vacation, sick, personal, etc). I've seen the sample forms and they aren't quite what I'm looking for. Does anyone have a form that they can share? Thanks in advance!
