Annual Performance Appraisal Form

I am looking to replace our complex annual performance appraisal form with a brief, simple yet effective document. Most of our employees are hourly but I'd prefer to use the same form for both staff and management.

If you really love your current form and would not mind emailing or faxing me a copy, please let me know.

PS - If your form is used in a bank or credit union, that would be a plus.

We have used our current review form for about 2 years now, and have been very pleased with the outcome. It focuses on both the employees' goals and aspiratins, asks for input regarding ways to improve our organization, and also has a set of performance criteria that the employee is measured against. It is not, however, short and sweet (contains about 8 pages, even more if you are in management) We take reviews very seriously and tell our managment team that each review should last a minimum of 90 minutes to do properly; some last over 3 hours. There is much discussion about our goals, strategies, profit expectation and methods we will use to attain them. It is not desingned for hourly or production type employees, but rather for administrative, professional and managerial folks. We have already seen overall improvement in communication among departments and improved employee and organizational performance.
