Annual Performance Appraisal Form

I am looking to replace our complex annual performance appraisal form with a brief, simple yet effective document. Most of our employees are hourly but I'd prefer to use the same form for both staff and management.
If you really love your current form and would not mind emailing or faxing me a copy, please let me know.
PS - If your form is used in a bank or credit union, that would be a plus.

We are a multi-specialty outpatient & hospital-based group of physicians so we not only need to streamline our process but also to develop something that we can use for evaluation of clinical and adminstrative (front desk, medical records) employees. I would love to see your performance evaluation. I want to do a goal based performance review with the employee also asking them to bring to the table accomplishments they would like to be recognized for in their appraisal.
