Employee Evaluations

Does anyone have an effective employee evaluation for support staff (secretaries, bookkeepers) that they wouldn't mind sharing with me? We are nearing that time of the year and find that ours is really outdated.

We use a 'canned' form for support staff. It's not in our system, but if you'd like a copy, send me your fax number.
