I think I noticed some examples of just what you're talking about on SHRM's site under Toolkits for recruiting. I would make sure to have your lawyer approve it before you use it to make sure it complies with FCRA requirements.

Another suggestion. Ask the company you use for background checks. Most of these have a sample lettter you can change to meet your needs. They usually have had these approved by counsel before distributing.

Any third party company conducting background checks is required by FCRA to provide a letter, such as you mention, to the person being researched. They will send you a sample.
