Waiver Form

We have a self funded Short Term Disability benefit. Does anyone have a waiver form for this? I have recently had a problem that an ee wanted to use her waiting period fromt he last time she was out for a disability, but did not fill out the required paperwork. When I prompted her to, she didn't think she needed it b/c she returned to work. Now she is having surgery for the same problem and feels that she should be able to use her time from previously toward her waiting weeks. I need a waiver form for ee's that decide they don't want to fill out the paperwork at the beginning. Thanks! [email]dhudak@hccadc.com[/email]
