certification of compliance

This one's for all you government HR folks.
For submitting financial data to our "parent" US Department, we also need to certify that we do not require employees to work over 40 hours per week without due compensation. It is a simple one page form requiring only a signature and date. I have seen this form somewhere either on the web or in a grant application in the past four years, but now I cannot recall it to mind. I've searched our Department's website and find nothing.
Does anyone know to what form I am referring?
