Our current self-feedback form has several questions like:
1. What would you say has been your best achievement since your last appraisal?
2. What is your biggest problem in doing your daily work?
3. Have any of your past problems in doing your daily work been solved? What were/are they?
4. What could I do to help improve your performance?
5. What would you like to see changed in your position/department over the next few months?
I am looking for some other questions that would elicit good useable responses from employees. I'd also appreciate self-feedback questions directed toward managers (or other exempts).

Thanks for your response.
Do you (or your managers) give the completed appraisal to the employees prior to the time when they sit down to discuss it or do they hand it to them the first time when they meet? I've heard pros and cons both ways. If they get it too early, they tend to focus only on the "coaching" or "negatives" and come in determined to defend themselves.
