Company evaluations

I am needing to use/find/develop a form that evaluates or rates our department/company by our employees. Has anyone used such a tool? Other than the usual compensation and benefits issues, what else should be addressed?

I must come up with an evaluation form, work with department heads to ensure all evaluations are completed by November 15, 2003. (All of the employees that will be evaluated are "administrative" employees)
