Reporting sexual harassment

Does anyone have a form to use when an employee wants to report an incident of sexual harassment?

Once a verbal complaint has been made, we ususally ask an employee to put their complaint in writing. A paragraph or two is all that's needed. We will begin to act on a verbal complaint though.
