Written policy regarding donations

I'm trying to create a "donation policy" that outlines who is authorized to approve donations exceeding some minimal amount. It would include a donation request form that would indicate such information as to the reason for the donation and any previous donation information if applicable. Other policy provisions to include would 'phone authorized credit card donations are not allowed'.

If anyone has a sample "donation policy" they would share with me I would greatly appreciate it.
