Peer Review Formats

I am currently researching what other companies use for their peer reviews. The one we currently use rates peers on 5 core skills based on a "Meets Standards" or "Below Standards" but then asks the reviewer to provide a brief comment. I am trying to find out if most companies just use a ratings scale only. I would think that comments provide managers and peers with some useful feedback on performance and that only rating 1-5 (for example) does not "force" a peer to really think about why they are rating a certain way.
