Criminal History Check form

To comply with the Federal Credit Reporting Act, apparently there must be a seperate form for applicants to sign that authorizes a criminal history check (or credit check) to be performed by a third party.

If the third party you are considering using is a good one, they will have their own form that they'll gladly supply you with. Their own form makes their job easier and more likely than not, is always absolutely legal and complete, for their own protection. Ask them to email you their form. If they don't have one, I'd look for another third party partner.
