Does anyone have examples of a Return To Work or Fitness For Duty form that they have found particularly effective? I am working on one for my current employer, and would appreciate any examples you may have.
I also would appreciate information on the process you use them in. Such as any instructions you provide physicians, or employees on how to complete the form; do you mail them directly to the physician, or does the employee hand carry? Those kinds of things.
