Form to document diciplinary action - verbal, written

Can I find a basic form to use to document supervisory meeting, verbal and written warnings for diciplinary action on HRhero.com or on the Internet?

  • Yes, we have sample forms you can access on this website, but only if you're a member of our Law Center in your state.

    Go to our main page by clicking on the HR shield at the top of this page. Then look at "Member Login" at the upper right. If you're not a Law Center member, click on "What are member benefits?" to see all the employment law publications and the Internet library you'd have access to as a Law Center member.

    If you're already a Law Center member, log in and you'll be at the Welcome page.
    - At the bottom of the Welcome page, under "HR Library," click on "Policies, Forms, and Manuals."
    - On the next page, click on "Forms."
    - On the next page, click on "GENERAL HR," and there you are.
  • You may want to modify the form to record who was present in the meeting. Remember, under the latest NLRB ruling all employers (union and non-union) must allow an employee to have a co-worker present if the believe that the meeting may lead to discipline. There are a lot of good articles on this, and if you do not already have an internal HR policy on this that you have communicated to your supervisors, managers and directors, you should do this ASAP.
