Authorized Pay Deduction Policy?

To conform with my state's wage payment and collections act, I am developing a wage assignment form to use in the case of misc payroll deductions or wage assigments outside of those allowed under the act. My question is, should I just develop this authorization form, or should I also amend our personnel policies to include a statement about how such authorized deductions could be made? Thank you.
