Benefit Change Notification Letter

My organization will be making changes to our medical insurance plan for our retirees. Does anyone have an example of a letter that I would send out to them explaining the who, what, when, and why? If so, could you please email it to me a [email]drehil@northlandcu.com[/email]. Thanks in advance.
