Reporting Personal Use of Company Auto

Would anyone be willing to share a form that employees use to track their use of both personal and business mileage if they are assigned a company vehicle?

Also, if you require personal mileage reimbursement (reimbursement TO the company), beyond de minimus personal use, do you use the IRS rate or a lesser rate?
