What is the time period for electing COBRA?

With the new COBRA forms that I am using, it does not state what period of time an employee has to "Elect" COBRA. I thought it used to be 60 days from the date of the qualifying event and 45 days from the date of election to make payments current. But the new forms don't say how long a person has to make the election. Is it still 60 days from qualifying date?

It is 60 days after the LATER of: the date the QB would lose coverage on account of the QE, or the date notice is provided to the QB of his/her rights to COBRA continuation.
