ERISA SPD Requirements

I'm looking for information on Erisa's requirements for distribution of SPD's. Can anyone provide a good link or more information. I located the DOL website and it gives the 60 day rule for benefit reductions and 210 day rule for general changes. The term "generally" is thrown in there which leads me to believe their may be exceptions to these guidelines. Any help would be appreciated.
