I am doing some housekeeping and have a question about my Medical files.

1) I recieve weekly EOB statements. Should I be keeping those in the Medical Files?

2) What about Life, STD, LTD enrollment forms. Should I put those somewhere seperate?

3) When someone terms...do I just put all insurance enrollment forms (LTD, STD, Life) forms in the medical file? What about the 401(K) file I kept seperately?
