Urgent! Health plan changes are coming

We're in the process of making design changes to employee medical insurance in order to reduce the impact of our renewal (24%). We want to obtain employee input about their utilization and the plan levels/options that are most important to them (to try and minimize the impact on morale).
We're drafting a survey to help with collecting this information (and have about 10 days to complete the process). If anyone has done a similar survey and is willing to share it, we'd really appreciate it!!! It can be emailed to [email]mbmecr@yahoo.com[/email] or faxed to 660-886-6724.
