short-term disability

We are trying to implement a self-funded short-term disability program. I am looking for how to structure the plan: lenght of employment to qualify; payments based on length of service and then a % of pay for up to 90 days, e.g. 1-5 years 70% of pay; or payments based on length of service for a defined # of weeks, e.g. 1 week of std per year of employment?
