Has anyone ever gotten an employee to sign a release for an injury that was non work related> if so, do you have a sample of form you could send?
We had an employee hurt his shoulder at home. Was very up front about it . Had surgery which doctor told them it may or may not work. We just want to have something in our files to cover ourselves down the road.

To be honest, and, again, I am not sure what you want the employee to release, I wouldn't sign a release.
