FMLA Forms?

I have an employee who is asking about FMLA. Her Father is sick and I believe she will be asking for leave to take care of him. I know she has to give notice but should it be done on some type of company form? or can she just write something up and sign it?

We modified our vacation and sick leave request form to add a check box regarding FMLA. However, no matter how much training or written material you provide EEs, many still do not get that their leave requests may qualify under the Act.

Make it easy for her to request, and give her the DOL medical certification form up front - for the provider to complete.
