Extended Work Comp Leave

I'm looking for guidance on what other companies do with employees on extended work comp leave. We accommodate restrictions as best we can; still, we have some injured employees who use up all FMLA time (or aren't yet eligible) and/or paid time off. In the past, if they couldn't return to work, we'd terminate; however, we're now looking to draft a "work comp policy" that addresses these long term situations, while we're awaiting MMI/PPI.
What does your company do? How long do you keep them as active employees? Do you continue benefits, offer COBRA, etc?
I'd appreciate any insight!
