Employee Choice of Dr Form

In Nebraska, this is called a Form 50. I had never seen it until recently. Does anybody else have any experience on these? Do employees fill them out right away after an injury, upon hire, or when? How are changes handled - do you usually approve unless there are circumstances?

The "Choice of Dr." form needs to be completed by the employee as soon as possible after each accident/illness.
When the form was first issued, all employees had to have the form signed prior to an injury. The WC law has changed since then.
