Doument Accomodations

JM in ATL

I have an employee who has a chronic condition which limits her dexterity in her arms and hands.

We have accomodated her needs for a special mouse and keyboard, and now she is requesting a special chair. We are going to provide the chair for her as well b/c it is a reasonable accomodation.

My question is would you document these accomodations and if so, do you have a particular form you would use or just put a written memo in the ee's personnel file?

I have done limited research on this and have not come up with an answer yet, so I thought I would add you guys to my search. X:)

The few accommodations I have dealt with have been recorded in the employee's medical file and include any information that led to the accommodation, the decision and any cost involved with a copy of the invoice.
