Physical Requirements

Does anyone have a Physical Requirements Worksheet they'd like to share with me? I have a W/C case that appears to be malingering so a nurse case manager has been assigned and I need to send her the physical requirements of the job for the injured employee. Problem is, I don't have one and I have not been successful searching web sites for one. I'd recreate one but my memory is not photographic.....
